I'm looking for a source that can create custom brass name plates for guitar cases. All I've found thus far are suppliers that require huge minimum quantities.

I have taken a name plate like that (Cedar Creek-Martin etc.) flipped it over, polished out the back (sand to 2000, micro mesh, and the wheel does it) and it's ready to be engraved. The ones I did were screwed on so it was easy. Rivets-not so easy.

Exactly, there must be a handful in Cowtown. Our local guy here will do 1 or 20 pretty reasonably.

I have used a friend who does laser engraving. The pic below is anodized aluminum ( any color you want) but he does do other materials including brass. Very cost effective. I'm sure you can turn up a laser engraver in your area

Most jewelers a will be able to do it... Used to work for one in Philly (one of many on jewelers row) there were a Handful of engravers on the row; all very reasonable (around $5 for a single item, cheaper for bulk)
